You are fully responsible for all fees charged by this office regardless of your insurance coverage. For your convenience, we provide a variety of payment options:
• Visa, MasterCard and Discover
• Personal checks approved by our Telecheck auto-debit machine
• Financing through CareCredit or Chase Health Advance
For our patients with dental insurance, we are happy to file those claims for you. Your co-pay and deductible are due at the time of service. Please remember that verification of insurance benefits does not guarantee payment. If your insurance company denies the claim and/or does not pay in full within 60 days, we require you to pay the balance due.
