Your initial appointment will either consist of the procedure itself or a pre-operative consultation explaining your diagnosis and treatment options. For routine extractions and wisdom teeth removal, many patients prefer scheduling the surgery without a pre-operative visit. For more detailed or complicated treatment plans, a consultation appointment may be required. A pre-operative consultation will be welcomed and encouraged by any patient desiring a visit with the doctor for any reason.
Checklist of items to bring with you to your appointment:
• Copy of your medical and dental insurance cards, if available
• Copy of guarantor's driver's license (the guarantor is the person financially responsible for the account)
• Medication list, if applicable
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian.
For patients undergoing IV anesthesia for surgery, please have nothing to eat or drink 6-8 hours prior to your surgery. You will also need a responsible adult to drive you home and be with you at home after your procedure.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.).
